The Ohio Department of Taxation has assembled the following links to cities and villages that have enacted a municipal individual income tax. The links below were supplied by the municipalities themselves, and are intended to help taxpayers find out more information about the tax or get access to tax forms.

In most cases, questions about these taxes should be directed to the city or village that enacted the tax or to the agency that administers the tax on the municipality's behalf. In a few cases, the Ohio Department of Taxation has agreed to host tax forms for a municipality. If a municipality is not listed below, the Department is not aware of a Web site for that city or village.
